Retail Theft/Drive-off Report

This form is for businesses to report a retail theft or fuel drive-off in which there is no meaningful and specific suspect information or the license plate number of the suspect vehicle is unknown or wrong.

This form is to report a retail theft in which the total dollar amount of the respective stolen items does not exceed $300 or the amount of the fuel stolen does not exceed $150.

Please be as specific as possible, adding any information you believe may be relevant to the incident.

For incidents where the aforementioned criteria is not applicable either by exceeding the dollar value or an incident in which there is meaningful and specific offender information/vehicle information, a police officer will come take a police report in-person.
